We provide Adult Foster Care which is a Mass Health funded program for eligible members. It provides in-home support to those who may have a medical or cognitive condition requiring assistance in the home.
Support may include
Active supervision/assistance to ensure the members well-being
Physical assistance with bathing, dressing, toilet, mobility or eating
All assistance is provided by a live-in caregivers
Caregivers
AFC caregivers are thoroughly pre-screened as qualified family members or non-related individuals. Caregivers must be at least 18 years of age, responsible and capable of making precise decisions. Qualified caregivers are paid a monthly stipend by the agency.
Program Eligibility For AFC.
Program participants must meet ALL of the following criteria: 16 years of age or older.
Insured by Mass Health Standard or Commonhealth.
Must require assistance with ADL's and IADL's daily.
